In this lecture, Professor Zach Murphy presents part 4 of our neuromuscular junction series, focusing on key pathologies that can impair neuromuscular transmission. We start with an overview of standard synaptic signaling, including acetylcholine release, receptor activation at the motor end plate, and subsequent excitation-contraction coupling. With this foundation, we explore how specific disorders disrupt these steps and lead to characteristic clinical presentations. We discuss autoimmune conditions such as myasthenia gravis, where antibodies target postsynaptic acetylcholine receptors, and Lambert-Eaton myasthenic syndrome, in which antibodies impair presynaptic voltage-gated calcium channels. We also cover the mechanisms of botulism toxin, which blocks acetylcholine release, and tetanus toxin, which inhibits inhibitory neurotransmitter release in the central nervous system. Additional conditions, such as organophosphate poisoning and certain pharmacologic agents, are explained to show how they alter synaptic function, leading to weakness, paralysis, or sustained muscle contraction. Finally, we connect these pathologies to their real-world implications by reviewing hallmark symptoms, diagnostic approaches, and the underlying pathophysiologic differences that help distinguish one condition from another.
