In this lecture, Professor Zach Murphy provides a detailed discussion of the dorsal column–medial lemniscus (DCML) pathway, one of the major ascending sensory tracts of the spinal cord. We begin by reviewing the types of sensory information carried in this pathway, including fine touch, vibration sense, and conscious proprioception. The lecture describes how these signals originate from mechanoreceptors in the skin, joints, and muscles, and travel through first-order neurons whose cell bodies reside in the dorsal root ganglia. From there, we trace the anatomical course of the pathway, starting with entry into the dorsal columns, where fibers ascend ipsilaterally within either the fasciculus gracilis (carrying input from the lower body) or the fasciculus cuneatus (carrying input from the upper body). We then discuss how these first-order neurons synapse in the nucleus gracilis and nucleus cuneatus of the medulla, where second-order neurons decussate as internal arcuate fibers to form the medial lemniscus. The pathway continues through the brainstem to the thalamus's ventral posterior lateral (VPL) nucleus, where third-order neurons project to the primary somatosensory cortex in the postcentral gyrus for conscious perception.
