<description>Official Ninja Nerd Website: https://ninjanerd.org Ninja Nerds! In this respiratory physiology lecture, Professor Zach Murphy explains Ventilation-Perfusion (V/Q) Coupling, a key component of external respiration that ensures efficient gas exchange between the lungs and blood. We begin by reviewing the basic physiology of ventilation (air reaching the alveoli) and perfusion (blood reaching the alveolar capillaries), and how matching these processes is critical for optimal oxygen and carbon dioxide exchange. The lecture emphasizes the normal V/Q ratio and how it varies from the apex to the base of the lungs due to gravity and regional differences in lung mechanics. You’ll learn about common causes of V/Q mismatch, including shunting (perfusion without ventilation) and dead space ventilation (ventilation without perfusion), and how these patterns are seen in clinical conditions like pulmonary embolism, pneumonia, COPD, and atelectasis. Minor Correction 27:50 - The ventilation INCREASES as we go down from the Apex to the Base, but the perfusion increases SIGNIFICANTLY more. Since Perfusion increases more than Ventilation as we move to the base of the lung, the V/Q ratio decreases at the base.
