<description>Official Ninja Nerd Website: https://ninjanerd.org Ninja Nerds! In this neuropathology lecture, Professor Zach Murphy provides an overview of Intracerebral Hemorrhage (ICH), a life-threatening form of stroke caused by bleeding directly into the brain parenchyma. This lecture connects the underlying pathophysiology to clinical presentation, imaging findings, and evidence-based management strategies. We begin by discussing the most common causes of ICH, including chronic hypertension, cerebral amyloid angiopathy, vascular malformations, anticoagulant use, and trauma. You’ll learn how hypertensive hemorrhages often affect deep structures such as the basal ganglia, thalamus, pons, and cerebellum. In contrast, lobar hemorrhages are more often associated with amyloid deposition in elderly patients. The lecture walks through the hallmark symptoms of ICH—sudden-onset headache, vomiting, altered mental status, and focal neurologic deficits—and explains how the location of bleeding influences the clinical picture. We emphasize the role of non-contrast CT in rapid diagnosis, along with key findings such as hematoma location, size, mass effect, and potential herniation. Management principles are reviewed, including blood pressure control, reversal of anticoagulation, monitoring for increased intracranial pressure, and indications for surgical intervention in select cases.
