We begin by breaking down the two major classes of receptors responding to acetylcholine: nicotinic and muscarinic. We discuss their structural differences, activation mechanisms, and distribution throughout the autonomic nervous system, neuromuscular junction, and central nervous system. You will learn how nicotinic receptors function as ligand-gated ion channels and mediate rapid depolarization, while muscarinic receptors act via G protein–coupled receptor pathways to produce a range of excitatory and inhibitory effects depending on the subtype involved. We then explore each muscarinic receptor subtype (M1–M5), emphasizing their specific tissue locations and physiological effects, such as glandular secretion, smooth muscle contraction, cardiac rate modulation, and roles in the CNS. For nicotinic receptors, we compare those found at the neuromuscular junction (Nm) with those in autonomic ganglia (Nn), detailing their respective roles in voluntary muscle control and autonomic signaling. Finally, we integrate these concepts into clinical relevance by examining how cholinergic receptor agonists and antagonists affect these pathways. We discuss examples such as neuromuscular blocking agents, atropine, pilocarpine, and the role of cholinesterase inhibitors in enhancing receptor activation.
